In a heartbreaking situation, a man in Fairfax, Virginia died on Wednesday after his dog’s leash became caught in a Metro train door causing the man to be dragged from the train platform to the tracks.

According to the Washington Post, the accident occurred at the Dunn Loring Station. The man had left the train and had been walking on the platform, but the dog’s leash appeared to be tied to him and became entangled. The dog stayed in the car, while the man was dragged off the platform and onto the tracks.

Surveillance video showed the incident happened about 450 feet away from the operator cab, and the train operator had allegedly performed two safe door checks before the train started on its way.

The man later died from injuries at the hospital. He has been identified as Harold Riley, 50. The dog. Daisy, is a service animal, had been left on the train. Reportedly Daisy had been wearing her service vest. Only service animals are allowed in a secure carrier on Metro.

Daisy has been adopted by another family because Riley’s daughter could not keep her.
